Back To School Forms

Dear Parents

In an effort to reduce the amount of paper and printing required during the Back-To-School registration process, we have made several forms available online.  The forms listed with an asterisk (*) can be filled out and submitted online.  The additional attachments can be downloaded and printed, but you only need to complete and return the forms that are pertinent to you.  Please review each section; the District Level Forms as well as the site-specific forms, and submit or return the necessary forms to your child's school.  Online forms need to be submitted by the first day of school and printed forms need to be turned in on the first day, August 24th.
 
Note:  We would like to have a Free and Reduced Lunch application submitted by each family.  Even if you do not qualify or choose not to participate, the information is critical for the district to have as we need to report demographic information to the state.  Additionally, if you are applying for Free Bus Transportation, submission of the Application for Free and Reduced Lunch is required.
