Volunteer Coach Packet

 

Volunteers must visit the district office to pick up TB test information and fingerprint information.

This Packet includes:

-Voluteer Form

-Drug and Alcohol Free Workplace Acknowledgement

-Required trainings for coaches (Mandated Reported, Sudden Cardiac Arrest, Concussion)

-Coach Code of Conduct
